Policies and Procedures

Appointments
We ask that you contact us as far in advance as possible to enable scheduling the most convenient day and time for you. We can also coordinate family member appointments so unnecessary trips are avoided.

New patients are encouraged to arrive at the office 15 minutes before the scheduled time to enable the staff to complete the registration process before you see the doctor. New patients may download the Patient Registration and Health History Form that can printed, filled out and brought with you for your appointment.

Cancellations
If you are unable to keep a scheduled appointment, please notify our office as far in advance as possible. We do have a minimum of 24 hours notice to avoid a cancellation charge.

Emergencies
If a problem arises which requires emergency care, Dr. Cogguillo is available during the day. If the problem occurs after hours, directions for emergencies are given by calling our office.
